Dear Parents/Guardians,

Welcome to Canton School System.  In order to meet state and district requirements, all children must have on file the following forms before they will be allowed to begin school.

Academic Requirements:

• Birth Certificate (original with raised seal) The school will keep copy and return the original to you.
• School Registration Form  
• Proof of residence (utility bill or mortgage statement/rent receipt)
• Academic records from previous school (if applicable)
• Special education records (IEP) from previous school (if applicable)

Health Requirements:

• Blue State of CT Physical Form 
  • Students entering Kindergarten, grades 7 and 11:must show proof of a physical examination, signed by a physician, given within eighteen months of the current school year.  Immunizations must be done as required by law at the time of the examination, with the results noted.
  • Transfer students:  entering any other grade, must show proof of a physical examination, including the results of a tuberculosis test, and immunizations as required by law prior to registration.
• Proof of immunizations per CT requirements
• Tuberculin test date and results, at least one required for all entering students
• Medical Questionnaire form
• Emergency Card
• Other pertinent medical records from special medical consultations

Please bring this document to the school office before expecting your child to begin school.  A careful review of records will help the school staff to provide a safe, healthy and appropriate educational experience for your child.
