Requirements
for student entering 7th grade
All students entering 7th grade are required by state law to have
a physical exam. The attached blue health form should be filled out
and must be handed into the nurses office before the first day of school
next fall. The State of Connecticut has also changed the immunization
requirements for 7th graders, you must show proof of:
- 1 hepatitis B vaccine. The series must be completed to enter 8th grade
the following year.
- 2 doses of measles vaccine
- Immunity to Varicella (Chicken Pox): the students must have a statement
signed by a physician, physician assistant, or an APRN indicating that
the child has already had chicken pox based on family and/or medical
history. OR 1 dose of the varicella vaccine
In the event of extreme financial hardship, please contact the Superintendent
of Schools, at 693-7704 for assistance. Pupils who receive school lunch
benefits are eligible for school-sponsored physicals. Please call as
soon as possible so necessary appointments may be made in time.
No child is required to undergo a physical examination, receive medical
treatment or receive medical instruction, provided that the parent
or guardian notifies the child’s principal in writing that the
parent or guardian objects on religious grounds or if a physician says
it is medically contraindicated.
Medications: Before
any medication, prescribed or over the counter, may be taken by
students at school, an Authorization for Medication form must
be on file in the school health office. No prescribed or over the
counter drugs (Tylenol, Advil, etc) will be administered by the
nurse or self-administered until this form is on file. This is
in compliance with Connecticut statutes and Canton Board of Education
policy. If you wish to have your child take medication of any kind
during school hours, please have your physician complete the enclosed
form and submit it to the health office. This policy is in effect
for the protection of your child and we do appreciate your cooperation.
Sports
Physicals: Physical examinations and clearance for sport
participation will be granted if the physical exam is within
1 year of the sport
the child is playing. If your child will be playing sports during
the school year, I strongly recommend you schedule your appointment
for after June 15, 2005.This will cover your child for the entire

Important Health Information
State Mandated Hepatitis B Immunization
Requirements
All
students entering 8th grade in the fall, must have written proof
with the completion of the Hepatitis B vaccination
series.
This series of immunizations should be completed during the
7th grade year.

Health requirements for entering 11th grade:
All
students entering 11th grade are required by state law to

will also cover interscholastic sport requirements for the
year. Your child may need a tetanus booster at this time. The booster
is given
every 10 years or sooner if there has been an injury. This
exam must be completed
and the blue physical form must be returned to the health office
before the 05-06 school year begins. My recommendation is to make
your appointments
for this summer now. Many physicians become very busy with
summer exams.

New Meningococcal Vaccine Require For College
Dear
Parents/Guardians,High school students planning on continuing their
education
at colleges or
universities in Connecticut should be made aware of the new meningococcal
vaccine requirement. Please contact your primary care provider
to inquire
about the availability of the vaccine,particularly if they intend
to live in campus housing. Attached you will find a copy of the
state
regulation and information on the vaccine.
