Please note the attached “Authorization
for Medication Form”. We will need this form filled out by
your physician and you. Please return the form to us as soon as possible.
This
medication order is necessary to have for over the counter medications
as well as prescription meds. These orders are to be updated yearly.
Students
are allowed to carry medication with them in grades 7- 12 as long
as it is reported to the nurse’s office. The exceptions are
if it is a controlled medications, this must be brought into nurses
office
by
an adult and given to the student by the nurse at the scheduled time.
For those students with allergic reactions to insect bites or foods,
please make sure the orders are specific as to what is to be given.
Please plan on meeting with the school nurse if orders are for severe
allergic
reactions.
